What the template covers

ApprovalScope, time zone, decision authority, and stakeholder sign-off.
Pre-change evidenceInventory, recovery path, package integrity, connectivity, alert routing, and notices.
Pilot and rollout gatesCheck-in, identity, telemetry, automation, wave reconciliation, and go/no-go decisions.
Rollback and closureStop conditions, reversion actions, communications, exception ownership, and final evidence.

How to use it

  1. Copy the CSV into your spreadsheet, ticketing system, or change record.
  2. Replace generic owners and thresholds with named people and measurable values.
  3. Record UTC start and end times, evidence links, and an explicit decision owner before execution.
  4. Do not begin the next wave until the prior wave reconciles and the go/no-go owner records a decision.
  5. Keep failed, excluded, offline, and unknown endpoints open as owned exceptions after the window closes.

This is an operational planning aid, not a substitute for your organization’s change-control, security, backup, or incident-response requirements.

Field reference

The CSV uses consistent fields for phase, control ID, activity, owner, planned times, success criteria, evidence, rollback trigger, rollback action, decision owner, communication audience, status, and notes.
